Responsibilities include medication administration, health monitoring, wound care, patient education, and coordination with physicians and families.

• Active, unrestricted RN or LPN license in the state of Massachusetts

• Current CPR/BLS certification

• Minimum of 1 year of clinical nursing experience; home care experience preferred

• Strong clinical assessment and critical thinking skills

• Excellent communication skills and ability to collaborate with patients, families, and interdisciplinary care teams

• Ability to work independently and manage time effectively across multiple client visits

• Comfortable with electronic documentation and care planning tools

• Physical ability to lift, transfer, and assist clients as needed, and to travel between client homes

• Valid driver's license and reliable transportation for travel throughout Boston and surrounding areas
